At Primed Potential, Educational Psychology PC, we understand that your information is personal, and we are committed to protecting it. We create and maintain records of the services you receive, including educational assessments, executive function coaching, and consultation. These records are necessary for providing quality services and meeting legal and ethical requirements.

  • Follow the terms of this Notice while it is in effect

We reserve the right to change the terms of this Notice. Any revised version will apply to all information we maintain and will be posted on our website.

For Services, Payment, and Operations

We may use your PHI to provide services, manage billing, and support business operations. Examples include:

  • Preparing reports following an evaluation

  • Coordinating services with another professional (with your consent when required)

  • Scheduling, accounting, or quality improvement activities

We may disclose PHI if required by federal, state, or local law, such as:

  • Reporting suspected child, elder, or dependent adult abuse or neglect

  • Preventing or responding to a serious threat to health or safety

  • Complying with a valid court order, subpoena, or other lawful process

1. Right to Access and Obtain Copies

You may inspect or request an electronic or paper copy of your records, with limited exceptions (e.g., test materials protected by copyright).
We will provide copies within 30 days of receiving your written request. Reasonable, cost-based fees may apply.

2. Right to Request Restrictions

You may request limits on how we use or disclose your PHI.
If you pay for a service out of pocket, you may request that information not be shared with your health plan.

3. Right to Confidential Communications

You may request specific ways for us to contact you (for example, by email only or at an alternate address). We will honor reasonable requests.

4. Right to Amend

If you believe your record is inaccurate or incomplete, you may request an amendment in writing.
We will respond within 60 days. If we deny your request, we’ll provide an explanation and allow you to submit a statement of disagreement.

5. Right to an Accounting of Disclosures

You may request a list of disclosures made of your PHI, excluding those for treatment, payment, or operations.
The list will cover up to the past six years and will be provided within 60 days.

6. Right to a Copy of This Notice

You may request a paper or electronic copy of this Notice at any time, even if you received it electronically.
